Grapefruit juice

· Severity: Major

· Notes for Consumers: Do not drink grapefruit juice or eat grapefruit without talking to your health care professional while you are receiving Ado-Trastuzumab Emtansine. Side effects from Ado-Trastuzumab Emtansine may get worse. Contact your health care provider if you experience nausea, bruising, pinpoint red spots on the skin, black, tarry stools, blood in the urine, headache, dark yellow or brown urine, general ill feeling or flu-like symptoms, light-colored stools, loss of appetite, right upper belly pain, unusually weak or tired, yellowing of the eyes or skin, or constipation.

· Notes for Professionals: Avoid administration of ado-trastuzumab emtansine with grapefruit juice due to the risk of elevated exposure to the cytotoxic component of ado-trastuzumab emtansine, DM1. The cytotoxic component of ado-trastuzumab emtansine, DM1, is metabolized mainly by CYP3A4 and to a lesser extent by CYP3A5; grapefruit juice is a strong CYP3A4 inhibitor. Formal drug interaction studies with ado-trastuzumab emtansine have not been conducted.
